Relocation Policy

1. Moving Expenses. The Clinic will reimburse Provider for normal household moving expenses for relocation. “Normal household moving expenses” shall include usual and standard handling and relocation of Provider’s immediate family’s personal belongings and usual household items to the practice area as defined in the Provider’s employment agreement. Provider shall be responsible for paying any applicable state and federal taxes on such payment. Relocation of motor vehicles, farm equipment, livestock, business equipment, etc. is not covered; neither is the expense of interim storage or relocation to any non-Holzer geographic area. Moving expenses to be reimbursed must be presented for payment by Provider no later than one year from the date in which such expense was incurred.
2. Expenses NOT covered under this policy include, but are not limited to:
   1. Apartment/house cleaning services
   2. Cleaning supplies
   3. Luggage purchases
   4. Dolly purchases
   5. Utility bills
   6. Room service and/or other additional temporary housing charges
   7. Childcare
   8. Maintenance and depreciation of vehicles
   9. Costs of selling or settling and unexpired lease
   10. Non-licensed lodging expenses (e.g., family residence, friend’s residence, etc.)
   11. Payment for labor furnished by friends or family
   12. Costs associated with selling a current residence
   13. Costs associated with buying a new residence
   14. Personal charges on hotel bills such as movies, dry cleaning, gym use, etc.
   15. Non-licensed moving companies
   16. Lodging, airfare, mileage and all other costs incurred outside of the shortest route from former residence to new residence
   17. Storage of household goods
   18. Temporary housing
   19. Transportation of:
       • Motor vehicles
       •  Farm equipment
       • Business equipment
   20. Relocation to non-Holzer geographical area or non-primary residence
   21. Meals
